Our office follows all applicable Federal civil rights laws and is committed to providing care without discrimination. We do not treat people differently or deny services based on race, color, national origin, age, disability, or sex.
Services We Provide
To ensure equal access and communication, our office offers:
· Free aids for individuals with disabilities, such as qualified sign language interpreters or information in alternative formats (large print, audio, accessible electronic files, etc.).
· Free language assistance services for people whose primary language is not English, including qualified interpreters, translation support, and written materials in other languages.

Filing a Complaint
If you believe you have been discriminated against or that our office has not provided the services described above, you may file a grievance with our Doctor. Grievances may be submitted in person, by mail, fax, or email. If you need help preparing a grievance, our Doctor and staff will assist you.
